Patient Authorization Form

This form is for your patients to authorize the use of their health care information in relation to support with EASE. This form can also be signed digitally through Docusign.

Dose Exchange Program

Provides a free 15-tablet supply in the lower dose to help patients who require a dose reduction.†§

30-Day Free Trial Program

Provides a free trial to help new CABOMETYX patients start treatment quickly, regardless of insurance type, with a 30-day additional supply available for patients with a payer decision delay of 5 days or more.*‡

At your request, EASE Case Managers can help provide assistance to finding coverage

  • Benefits Investigation Support: Review patient’s insurance coverage and health plan requirements 
  • Prior Authorization Assistance: Identify authorization criteria and obtain and/or provide information on any necessary paperwork required by a patient’s health plan for services related to treatment
  • Appeals Support: Help review reason for denial(s) and with understanding any appeals guidelines related to a patient's health plan
  • Follow-ups and Status Updates: Check on status of any authorization, appeals or approval of coverage, and provide status updates

The completion and submission of coverage- or reimbursement-related documentation are the responsibility of the patient and health care provider.
