Eligible patients who cannot afford their drug costs may receive CABOMETYX free of charge.^†

You can help enroll your eligible patient in this program by completing the EASE Enrollment Form via Docusign or by downloading, completing, and faxing back to 1-844-901-3273.

The Patient Authorization Form will also need to be completed, either via Docusign or by downloading, completing, and faxing back to 1-844-901-3273.
