Your patient can also enroll themselves in the Co-Pay Program online at www.activatethecard.com/exelixis/#.

Eligible patients who cannot afford their drug costs may receive CABOMETYX free of charge.†
You can help enroll your eligible patient in this program by completing the EASE Enrollment Form via Docusign or by downloading, completing and faxing back to 1-844-901-3273.
The Patient Authorization Form will also need to be completed, either via Docusign or by downloading, completing and faxing back to 1-844-901-3273.
This form is for your patients to authorize the use of their health care information in relation to support with EASE.
